By initialing below, I hereby make application for membership to the Ames Golf & Country Club and understand that this application is subject to approval of the Board of Directors.

It is expressly understood that this application is subject to the Initiation fees and dues in effect for new members at this time of my admission to the Club. Upon acceptance, I agree to abide by the By-Laws and Rules of Conduct of the Ames Golf & Country Club.

I agree to belong to the Club for at least 12 months and to pay dues and minimum at rates determined by the Board of Directors. I understand that I may resign from the Club on condition that I have tendered my resignation in writing to the Secretary of the Club at least fifteen days prior to the first day of the month in which the resignation is to become effective and paid all indebtedness owed to the Club.

Your membership will be effective after approval from the board, and the initiation fee has been received.
